ABOUT
For student athletes to participate in baseball, ALL required paper work must be filled out completely and returned to the head coach. These requirements are as follows:

1. Parental consent forms include: medical release and insurance information, concussion awareness, heat policy, and handbook acknowledgement.

2. All student/athletes must have a physical signed by a doctor to try out for baseball. By Georgia law, this must be on the GHSA physical form. Coaches should keep a copy of this form, and the original is kept in the athletic director’s office. A physical is good for one calendar year.

3. Eligibility reports should be turned in to the athletic director or academic coach before final cuts are made. All players must be GHSA eligible. CHS eligibility requirements are: Freshman must pass 3 out 4 classes fall semester, 10th graders must have 5 credits, 11th graders must have 11 credits, and 12th graders must have 17 credits.

4. Only the signed physical form may be used from one sport to another. All other forms must be signed for each sport the student athlete participates in.

5. Athletes will not be allowed to quit one sport to begin another until the previous sport had concluded either its regular or post season schedule.

6. All participation requirements are to be in the head coaches’ possession before tryouts begin.
